Registration/Waiver form

Complete and sign the enclosed registration form and return it to OHFS, along with the deposit, by February 22, 2007.

NOTE:  Everybody will need to fill out a registration form, no matter if they make hotel reservations or not.  This will assist us in scheduling workshops, performances, excursions etc.
